[OSHA 29 CFR 1904.39] How soon must I report an employee's death or hospitalization?
All employers must report to OSHA any workplace incident that results in a fatality
or the hospitalization of three or more employees. According to

CFR 1904.39(a), employers must report to OSHA within 8 hours the death of
any employee or the in-patient hospitalization of three or more employees as a
result of a work-related incident. Employers must report the fatality or multiple
hospitalizations by telephone or in person to the local OSHA area office that
is nearest to the site of the incident. You may also use the OSHA toll-free central
telephone number, 1-800-321-OSHA (1-800-321-6742). A written report must be filed
within 30 days (
